Medicare Part D Notification:
ANNUAL DISCLOSURE of creditable coverage status to CMS is required WITHIN 60 DAYS after the first day of the plan year (e.g., March 2 for calendar year plans). The online disclosure form is located at the CMS website:
https://www.cms.hhs.gov/CreditableCoverage/45_CCDisclosureForm.asp.

Employers offering a prescription drug plan must determine annually whether their prescription drug plan is considered "Creditable" or "Non Creditable" coverage.  Employers must send a notice to Medicare eligible associates by November 15th each year.  One way to meet this requirement is to include it with the open enrollment mailing.  The model language for Medicare Part D notices has been modified slightly.  The electronic delivery requirements were also significantly relaxed.  The model notices are available on the CMS website at: http://www.cms.hhs.gov/CreditableCoverage/.  The notices are available in both English and Spanish.
